BEHAVIORAL HEALTH MEDICAL ASSISTANT

The Behavioral Health Medical Assistant should be an empathic individual with competent observational, clinical, critical thinking, communication, problem-solving and decision-making skills to meet patient needs and contribute to a positive patient care experience and work environment. Takes part in the daily review and process of internal and self-referrals for behavioral services. Provides coordinated care to a variety of patients enrolled in Psychiatric and Counseling services with BCHC, Inc, and functions as part of the interdisciplinary care team by providing education and case management directly to patients, and support to Psychiatrists and Behavioral Providers. Performs intake triage, assists patients with mental health screenings, vitals, and referrals to labs. Coordinate with clinic managers and their front/back-office services regarding psychiatric and counseling services. Must have a minimum of one year of related experience. Medical Assistant Certification and experience working with persons in crisis is preferred.

 

CHIEF MEDICAL OFFICER

The Chief Medical Officer should be a practicing physician who is Board Certified in Family Medicine or Internal Medicine; 3+ years of experience in ambulatory health care leadership, preferably in an FQHC setting. The candidate should have experience in leading quality improvement/quality assurance activities; supervising physicians, mid-level providers, and nursing leaders; overseeing peer review activities; mitigating medical risks; recruitment and retention of physicians and supervisory clinical personnel; public speaking and community health education; and developing and implementing medical protocols, policies, procedures, and organizational plans. The CMO will assist the Chief Executive Officer with medical and administrative concerns that pertain to health policy, grant applications, and reporting.

This is a full-time position with evenly distributed time between administrative and clinical duties.

GRANT ADMINISTRATOR

The Grants Administrator will be responsible for all aspects of grant research and identification, grant proposals development and writing, and for submitting timely and accurate reports for all existing grant funded projects. This position supports funding of existing and planned program activities through grant funding. Must hold a Master’s Degree in Public Health Administration, Public Administration,or related field preferred. A Minimumof5 years project management experience and minimum 2 years of grant oversight experience, preferably government grants.

LICENSED MENTAL HEALTH COUNSELOR (LMHC) - BEHAVIORAL HEALTH

The LMHC must be a compassionate and experienced mental health counselor to provide our clients with professional care. In this role, you will be responsible for conducting mental health evaluations, developing treatment plans, and facilitating individual and group interventions. You may also be required to manage referrals. Must have a Master's degree in psychology or counseling (e.g., MA, MS, MSW, MFT, MHC) from an accredited program.

 

LICENSED PRACTICAL NURSE (LPN)

The Medical Assistant can specialize in either the Administrative or Clinical aspect of the job or be multi-functional. Competence requires communication skills, recognition and response to emergencies, adherence to ethical and legal standards, and demonstrable professional characteristics. PART C CASE MANAGER

The Part C case manager conducts patient health assessments, completes needs assessments on all patients referred for case management services. The case manager communicates assessment finding to the referring provider and/or nurse in a timely manner. The case manager will contact all parties involved in the patient’s plan of care for continual progress reports. This may include coordinating services with existing partners and new partners in the referral system. The case manager participates in multi-disciplinary meetings to ensure access to care. The case manager will prepare documentation to facilitate case conference information. The case manager interacts with other resources in the community, such as AIDS drug Assistance Program (ADAP), Medicaid and Social Security. The case manager (CM) interacts with and assures coordination of care by working with the provider and other medical professionals to develop a final treatment plan and with other Bond team members to ensure the patient is linked to all comprehensive services and community resources thereby assuring patient continuum of care. Must have bachelor's degree in a human service field or a degree in nursing; Master’s Degree preferred. Three years of case management experience working in the health care system with indigent populations. Three years of counseling experience, Prefer two or more years of experience with HIV populations. Possess the ability to work effectively with persons of varying degrees of education and professional experience in the multi-disciplinary team setting. Possess excellent written and verbal communication skills Possess the ability to work with minimal direction. Be comfortable working in multicultural environments.

 

PHARMACY TECHNICIAN

Must have current Florida Registered Pharmacy Technician License. Proficient in verbal and written communication with medical professionals and patients. Computer skills including pharmacy-based software programs, excel, and electronic health records. Ability to read, and interpret medical prescriptions, and progress notes. Able to perform basic pharmaceutical calculations including quantity, volume, and percentages. Physically able to stand for an extended period. Capable of complying with HIPAA and confidentiality standards. Able to work with minimal supervision. Ability to accurately count and maintain large sums of cash. Must work one Saturday per month and have the flexibility to work extended hours when necessary, including evenings and weekends.

SECURITY OFFICER

Bond CHC Security Officer is responsible for enhancing the safety and security of Bond CHC staff, patients, and visitors. This position will work towards minimizing the risk of loss to Bond CHC property. Provide access control as appropriate to sensitive areas as defined by each facility. TRANSPORTATION SPECIALIST - Mobile Unit

The Transportation Specialist is responsible for all phases of transportation operations for mobile health services. This includes skilled work in safe and efficient operations of a large passenger van, transporting mobile unit staff or patients to various scheduled appointments. Must possess a valid Florida Driver’s License, Class A, B, and/or CDL, and proof of driver records from the Florida Department of Motor Vehicles. Also supports maintenance team with facility work order requests, set-up and break down for special events, traffic control, and janitorial rounding. 1–2-year related experience required. Must have a high school diploma or equivalent recognized certificate.
